 Every three years, Ballad Health, in collaboration with our local health departments and community organizations, undergoes the Community Health Needs Assessment (CHNA) process. The CHNA process provides comprehensive information about our community's current health status, needs, and issues. This information will help develop community health improvement plans by justifying how and where resources should be allocated to best meet community needs, with the overarching goal of making the Appalachian Highlands Region a healthier place for all.

The Community Member Survey is a comprehensive data collection effort to identify the strengths, challenges, and opportunities to address and improve health in the Appalachian Highland Region.  The purpose of the survey is to develop a better understanding from residents about issues related to quality of life, availability of services, physical and mental health, and more! The information gathered from this survey is completely anonymous and will be used to identify the most pressing issues the Appalachian Highland Region is facing. 

The survey should take approximately 10-15 minutes to complete.
